Send a completed Form by E-mail or by Fax:
By E-mail: just save as copy of the completed form to your desktop. It will be saved as a file. You may want to create a temp folder on your desktop to put the file in. You can save an un completed form and fill it out later using the onboard typwriter.
You can also make a copy of a form and fill it out manually to mail to AAHS. If you have onboard print to a fax you can sent it through that method also.
